Please have any health insurance card, referrals, previous medical reports, or blood test results ready for your check-in, if you have any with you!

Please have the following items ready for your check-in:

If you do not have your referral slip with you, we cannot examine you.

Lab results for CT: Creatine and TSH no more than 4 months old!
